The Challenge

A Yuvo Health-affiliated Federally Qualified Health Center (FQHC) faced key challenges in delivering comprehensive, high-quality care and maximizing value-based care performance. Specifically:

  • Providers didn’t always have access to critical diagnostic and treatment information—especially following emergency department visits, hospitalizations, or other external care events.
  • This lack of visibility made it difficult to close gaps in care and accurately assess patient risk.
  • As a result, the FQHC risked underreporting the severity of patient conditions, potentially missing opportunities for appropriate reimbursement.
The Solution

Yuvo Health provided access to a suite of technology tools or their “tech flywheel” that is curated specifically for the FQHC environment. This included Vim Connect which sits within the EMR workflow to highlight high-priority care gaps and support more accurate coding and treatment. Providers and team members can now view claims data, patient health history, admissions, discharges and transfer (ADT) data and more from multiple sources directly within the EMR.
