Health Information Exchange (HIE) refers to the secure sharing of patient health information among healthcare organizations and providers. The goal of HIE is to improve patient care by ensuring that healthcare providers have access to a patient’s complete health history, including diagnostic test results, medications, and other relevant information.
HIE can be done through electronic methods, such as through a centralized database, or by exchanging electronic health records (EHRs) between providers. The use of HIE helps to reduce medical errors, improve patient outcomes, and reduce healthcare costs by avoiding duplicative tests and treatments.
The mechanisms for HIE vary and are often tied to federal funding and state or regional organizations, often referred to as Regional Health Information Organizations (RHIOs).
Vim Patient Health History expands a care team’s awareness of a patient’s recent health history – beyond the walls of their own clinic – using claims-based data. This application gives a window into a patient’s entire recent health history at the point of care without relying solely on a patient’s recall or wasting time re-writing (or retyping) history pulled from Health Information Exchanges (HIEs), portals, and other out-of-workflow sources.
